Diabetes Coordinator

Nisqually Indian Tribe

This position has regular contact with or control over Indian Children and Vulnerable Adults. In compliance with Federal Law, this position will not be filled by an applicant with any felonious offense or any of two or more misdemeanor offenses under Federal, State, or Tribal law involving crimes of violence; sexual assault, molestation, exploitation, contact, or prostitution; crimes against persons; or offenses committed against children.

NATURE OF WORK: The Diabetes Program Coordinator is responsible for administration of diabetes case management and program development to reduce the instances of diabetes and health complications including all objectives of applicable diabetes grant deliverables, public health policy and priorities of the Nisqually Tribal Health Center. Includes areas of diabetes prevention, nutrition, patient and staff education, patient and program assessment, patient self-management skills, and bridging patients to supplemental resources.

DUTIES AND RESPONSIBILITIES:

  • Participate as a member of the Health Center team to emphasize a multidisciplinary approach to patient care, provider and community education to develop, implement, and evaluate project plans, policies, procedures, and operations for the Diabetes program.
  • Oversee the Diabetes program for the Health Center and implement all deliverables of the Special Diabetes Program for Indians (SDPI) for the Nisqually Tribe.
  • Ensure compliance with all Federal, Tribal, State managed care standards, regulations, policies and procedures.
  • Oversee patient care coordination and data management plan for the Diabetes program; serving as a navigator for diabetes patients helping them understand medical tests, healthy eating and lifestyle.
  • Coordinate Diabetes program services within the Department and ensure appropriate Electronic Health Record and data retention, utilize electronic health records system to input date associated with the Diabetes Program.
  • Program level responsibilities include; program planning, implementation, coordination, monthly, quarterly and annual reports as necessary including program evaluation.
  • Provide public relations presentations and maintain linkages to key stakeholders, such as Indian Health Services.
  • Draft monthly newsletter articles, community mail out information, social media posts, and other forms of program communication.
  • Coordination of diabetes events and activities within budget guidelines.
  • Coordinate provision of continuing medical education to staff related to diabetes.
  • Coordinate and facilitate communications to advance knowledge of stakeholders.
  • Maintain and prepare records, documents and data for annual IHS and Tribal audits
  • Perform other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of Tribal, Local, State and Federal resources available to program clients and their families; knowledge of Indian Health Service programs.
  • Knowledge of Health Department, Tribal, and other related policies, procedures, and systems.
  • Knowledge of HIPAA and other laws, rules, and regulations related to the security and confidentiality of data.
  • Knowledge of electronic health records, systems, and related applications.
  • Knowledge of accounting and budgeting principles.
  • Skill in gathering and interpreting data.
  • Skill in planning, delegating, and supervising.
  • Skill in maintaining an organized work environment.
  • Skill in both verbal and written communication.
  • Ability to take on responsibility and exercise independent judgment, decision making, and problem solving expertise.
  • Ability to research and prepare reports or other correspondence as required.
  • Ability to demonstrate compassion and be proactive in assisting patients with their healthcare needs.
  • Ability to provide excellent customer service and exercise tact and good judgment in all contacts.
  • Ability to work with diverse teams of people.
  • Ability to multi-task and manage priorities.

MINIMUM QUALIFICATIONS:

  • Bachelor's Degree in Health Management, Project Management, Business Management or a closely related field.
  • 2 years recent related work experience.
  • OR a comparable combination of approved education and/or recent related work experience.
  • Must have a valid driver's license and be insurable under the Tribe's insurance policy.
