Social Services Director

Social Services Director RYZE West is seeking a dedicated and compassionate Social Services Director to join our leadership team. As a key member of our facility, you will be responsible for overseeing the Social Services Department and ensuring residents receive the emotional, psychosocial, and supportive services needed to enhance their quality of life. This role works collaboratively with the interdisciplinary team to promote resident well-being, protect resident rights, and facilitate successful transitions throughout the continuum of care. Responsibilities Develop and implement social service programs and services that meet the needs of residents and their families Collaborate with the interdisciplinary team to develop and implement individualized, resident-centered care plans Complete and oversee psychosocial assessments, social histories, and care plan documentation Lead, supervise, and support the Social Services team Coordinate discharge planning and facilitate safe transitions to the next level of care Assist residents and families with adjustment, counseling, crisis intervention, and access to community resources Ensure compliance with all federal, state, and local regulations governing social services in skilled nursing facilities Advocate for resident rights and promote dignity, independence, and quality of life Participate in Quality Assurance (QA), care plan, Medicare, and interdisciplinary team meetings Develop and manage the Social Services Department budget and resources Build and maintain relationships with community organizations, hospitals, and referral partners Monitor program outcomes and identify opportunities for continuous improvement Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements Qualifications Minimum of 2 years of experience in Social Work or Case Management in a skilled nursing or long-term care setting required Bachelor's degree in Social Work or a related human services field preferred Previous leadership or supervisory experience preferred Knowledge of CMS regulations, resident rights, discharge planning, and psychosocial assessments Strong communication, leadership, organizational, and interpersonal skills Ability to work collaboratively with residents, families, physicians, and interdisciplinary teams Compassionate, professional, and resident-centered approach to care Benefits Medical Insurance Dental Insurance Vision Insurance Flexible Spending Account (FSA) Company-Paid Life Insurance Critical Illness Insurance Short-Term Disability Insurance Accident Insurance Hospital Indemnity Insurance 401(k) Retirement Savings Plan On-Demand Pay Option Paid Time Off (PTO) Tuition Reimbursement Program Job Type: Full-time Location: RYZE West | Chicago, IL At RYZE West, we're committed to providing a supportive and collaborative work environment that fosters professional growth while delivering exceptional care to our residents. #J-18808-Ljbffr
