Hospice Registered Nurse Case Manager

Gentiva Hospice in Oak Brook, IL is seeking a dedicated Hospice RN / RN Case Manager to lead patients and families through the hospice journey with personalized, compassionate care in home settings. You will assess, plan, and document care, coordinating with an interdisciplinary team to refine plans and educate caregivers. The ideal candidate will have at least 1 year of RN experience, hospice or palliative care preferred, and a strong commitment to patient-centered, comprehensive care. #J-18808-Ljbffr
