Butler Rehab & Healthcare located 416 S High street, Butler, MO is seeking an   MDS/Care Plan Coordinator   to join our friendly team.

MDS Coordinator- Requirements:

• High school diploma or equivalent.
• MDS Experience: 1 year (Preferred).
• RN/ LPN License.

Responsibilities of  MDS/Care Plan Coordinator :

·        Actively participate in the development of a comprehensive, individualized plan of care for each resident.

·        Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care/needs.

·        Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions schedule

·        Direct, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursement

·        Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements

·        Participate in resident level of care determination upon admission

·        Review reports of timely completion of residents' MDS assessments and care plans and take follow up action as necessary

·        Analyze Quality indicator reports for potential resident concerns and problems and review residents' care plans to be sure this information is included

·        Will maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).
