Key Responsibilities\n

• Serve as a frontline clinician for clients facing major mental illnesses and severe emotional disorders, both in office settings and through community engagement.

• Conduct thorough assessments, screenings, and evaluations to determine individual client needs and tailor services accordingly.

• Create and implement integrated care plans, collaborating with teams and other providers to ensure continuity of care and optimize patient outcomes.

• Participate effectively in team meetings, sharing insights and strategies to enhance service delivery and engage in collective problem-solving.

• Provide individual, group, and family therapy, along with crisis intervention as necessary, to support clients and their families during challenging times.

• Monitor and document patient progress while ensuring timely reporting and compliance with established protocols and ethical standards.

• Participate in quality improvement initiatives and committees to contribute to the ongoing enhancement of agency services.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care, who may be experiencing terminal illnesses and require specialized support. This includes addressing the emotional needs of clients and their families during the difficult transition phase of end-of-life care. You will be expected to engage with diverse populations reflecting various cultural, ethnic, and socioeconomic backgrounds, ensuring an environment of respect and dignity.

Qualifications\n

• A master’s degree in social work from an accredited institution.

• Licensed as a Clinical Social Worker (LCSW) in the state of Florida.

• At least two years of clinical experience, preferably in a healthcare or hospice setting.
