Key Responsibilities

• Evaluate patients to understand their needs and establish a comprehensive Plan of Care, ensuring it aligns with medical necessity and adheres to recognized clinical guidelines.
• Implement diverse therapeutic interventions, taking into account individualized treatment modalities, length, intensity, and frequency of therapy sessions.
• Document clinical findings and treatment outcomes meticulously, generating reports that highlight areas for improvement and trends in patient progress.
• Collaborate with fellow therapists for peer consultations, particularly regarding complex cases or those lacking adherence to clinical appropriateness before escalation to the Medical Director.
• Conduct thorough reviews of patient requests for physical therapy services, analyzing supporting clinical information such as diagnosis, evaluations, and progress notes against established standards.
• Maintain compliance with all company policies and operational procedures to contribute to a seamless operational framework.

Patient Population / Client Focus

The Physical Therapist will work with a varied patient demographic, including children, adults, and elderly individuals requiring rehabilitation services. This position involves a substantial focus on patients dealing with physical injuries, cognitive impairments, psychosocial dysfunctions, and learning disabilities, as well as those recovering from surgery or managing chronic pain. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current and active license to practice as a Physical Therapist.

• A minimum of 2 years of experience in outpatient and acute care settings.
• Experience working with pediatric patients is preferred.

• Proven ability to provide comprehensive inpatient care alongside outpatient therapies.
• Experience in using electronic medical records (EMR) systems and data collection tools for patient documentation.
• Background in interdisciplinary collaboration with other healthcare professionals, emphasizing holistic treatment approaches.
