Work Environment\n

This role allows for a completely remote work setting, providing flexibility in your schedule while maintaining continuous care for our clients. Full-time hours are typically Monday to Friday, from 8:00 AM to 4:30 PM, although occasional evening or weekend hours may be necessary to accommodate client needs. On-call availability may also be required during off-hours to address urgent situations, ensuring our clients receive consistent support.

Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the psychosocial needs of clients, particularly seniors, taking into consideration emotional, mental, and social factors that affect their well-being.

• Collaborate closely with clients and their families to devise personalized care plans that reflect their unique goals, preferences, and circumstances.

• Provide counseling and emotional support, assisting clients and their families in navigating through transitions related to aging, mental health issues, and life challenges.

• Serve as an advocate for clients, ensuring they have access to essential community resources and receive the appropriate support services necessary for their recovery.

• Work alongside interdisciplinary teams, including nurses and healthcare professionals, to coordinate care, share insights, and enhance service delivery.

• Perform regular visits to monitor client progress, assess living conditions, and proactively address any safety concerns or issues that may arise.

• Facilitate communication among family members involved in the care of clients, resolving conflicts and ensuring a unified approach to treatment.

• Educate clients and their families about available community support systems, support groups, and programs that can enhance their social engagement and overall quality of life.

• Maintain precise documentation of client assessments, care plans, progress notes, and all relevant interactions, adhering to ethical standards of practice.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.
