Responsibilities

• Coordinate and oversee patient care plans in-home, ensuring timely care delivery.

• Assess patient needs, create and adjust care plans based on progress and metrics.

• Monitor clinical indicators and document outcomes to drive improvement.

• Meet or exceed individualized targets and contribute to team-based metrics.

• Collaborate with physicians, therapists, and families to align goals and care.

• Ensure compliance with regulations, policies, and safety standards in home health.

• Identify and escalate risks; implement preventive measures to maintain quality care.

• Document all interactions and care activities within electronic health records accurately.

• Utilize pipelines and territory data to forecast caseload and target outcomes.

• Provide patient and caregiver education to enhance self-management and adherence.

• Participate in performance reviews and contribute to professional development plans.

Experience/Qualifications

• RN license valid in the relevant state with Home Health certification.

• Proven home health care management experience coordinating patient care plans.

• Strong clinical assessment skills and ability to adjust plans based on progress.

• Compliance-focused with knowledge of home health regulations and safety standards.

• Proficiency with electronic health records and accurate documentation.

• Experience using pipelines and territory data to forecast caseloads.

• Excellent communication with physicians, therapists, families; interdisciplinary collaboration.

• Target-driven mindset with measurable performance metrics and accountability.

• Ability to educate patients and caregivers to improve self-management and adherence.

• Contract position availability with on-site work and reliability to meet targets.
