Job Summary

We are hiring a Licensed Bachelor Social Worker (LBSW) to assist patients with discharge planning, care coordination, resource referrals, and psychosocial support while working closely with the healthcare team.

Requirements

Bachelor's Degree in Social Work or related Social Services field
Active Kansas LBSW License
Hospital-based social work experience preferred
Excellent communication and case management skills

Pay & Benefits
