Key Responsibilities\n

• Conduct thorough psychosocial assessments through structured interviews, direct observations, and comprehensive reviews of medical records to identify emotional, social, and environmental factors influencing recovery.

• Provide empathetic counseling and practical support to patients and their families, focusing on adjustment challenges, emotional distress, and overall psychological wellness.

• Collaborate with the rehabilitation team to create personalized treatment plans that are aligned with each patient’s psychosocial needs and recovery goals.

• Identify and facilitate access to community resources, support groups, vocational rehab programs, and housing assistance as needed throughout the treatment process.

• Educate patients and their families on the availability of community resources and support services that can aid in post-rehabilitation transitions.

• Actively participate in discharge planning, helping to ensure a smooth transition back into the community by preparing both patients and families for home care requirements.

Your work will primarily focus on adult patients in a detox setting who are dealing with substance use disorders and other co-occurring mental health issues.
