Job Description
Title: Physical Therapist - Rehabilitation

Schedule: Days (8:00 AM - 4:30 PM), no weekends

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

Requirements:
- Current unencumbered state Physical Therapy license required.
- Healthcare Provider Level BLS required.
- Must be able to travel to multiple points of care.

Roles & Responsibilities:
- Evaluate and treat patients with injuries or conditions affecting physical mobility and functional independence.
- Develop and implement individualized rehabilitation plans to enhance patient recovery and return to community activities.
- Collaborate with healthcare team members to ensure comprehensive patient care.
- Utilize specialized techniques in orthopedic, neurological, and stroke rehabilitation, as well as chronic pain management.
- Monitor and document patient progress regularly to adjust treatment plans as necessary.
