Occupational Therapist

The Occupational Therapist (OT) provides occupational therapy evaluations and treatment plans to participants. All participant care provided is in accordance with applicable InnovAge policies and state regulations.

Responsibilities include:

• Providing clinical occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
• Assessing condition, creating a treatment plan and discussing implementation of therapy with participant or caretaker(s).
• Integrating occupational therapy treatment plan in the team Plan of Care (POC).
• Assisting in the ordering, inventory, distribution, and maintenance of durable medical equipment for participants.
• Making home visits/educational sessions as needed and indicated in the POC, and documenting activities including participant/caregiver response in participant's chart.
• Educating participants and family members in proper care and use of various supports and exercise programs in addition to the use of durable medical equipment (DME).
• Administering treatment program per referral from assessments from the interdisciplinary team (IDT)
• Instructing family members, caregivers, and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL) with documentation in medical record notes.
• Evaluating charts and reporting on participant's progress to appropriate staff.
• Documenting occupational therapy services provided and performing department record-keeping in keeping within documentation requirements.
• Performing data collection, preparing reports and maintaining any necessary participant statistics.
• Assisting in recommending policies and procedures in the area of occupational therapy.
• Assisting the physician or intermediate care provider in evaluating level of function, helps develop the plan of care and revise as necessary.
• Participating in in-service programs and presenting in-service programs as assigned.
• Participating with all other health care personnel in patient care planning.
• Performing all skilled occupational therapy procedures as ordered by physician.
• Writing reports to physicians regarding patient's progress.
• Attending all staff meetings and patient care conferences as scheduled.
• Completing a discharge summary upon completion of therapy services.
• Preparing and submitting patient evaluations, clinical and progress notes in a timely manner as outlined in Agency policy.
• Immediately reporting any patient incidents/variances or complaints.
• Participating in peer review and QM activities as requested.
• Participating in utilization review of medical records as assigned.
• Adhering to Agency standards and consistently interpreting and accurately performing all assigned responsibilities.
• Reporting all incomplete work assignments to appropriate member of clinical management staff
• Working in collaboration with InnovAge nursing home and assisted living staff regarding the occupational therapy needs of participants who are their residents.
• Conduct and participate in clinic educational programs.
• Performing data collection, preparing reports and maintaining any necessary participant statistics.
• Maintaining therapy department in a clean and orderly manner and overseeing the maintenance of therapy equipment.
• Maintaining the quality assurance, safety, environmental and infection control programs
• Fulfilling the documentation requirements and department record-keeping of OT services provided as set by InnovAge programs
• Attending Morning Meetings, mini teams and family meetings and forwarding all information to the Primary OT if no other OT is available or if assigned by the Primary OT
• Participating within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants.
• Performing other duties as assigned.

Required:

• Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA)
• Bachelors in (OT), Masters in OT (MOT), Doctorate in OT (OTD)
• National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
• Registration with state in which practicing
• Requires state issued driver's license, personal transportation, good driving record and auto insurance as required by law
• A minimum of one year experience working with the frail or elderly is required.

Preferred:

• Two (2) years' experience, preferred
• A minimum of one year of general occupational therapy experience in a geriatric setting preferred
