Clinical Case Manager II - Intensive Case Management (ICM)
Intensive Case Management
Queens Supervised Release Program
The New York City Criminal Justice Agency (CJA) is a not-for-profit service and research organization that operates in partnership with the New York City Mayor's Office of Criminal Justice. CJA is the City's main pretrial services agency, combining operations, pilot projects, and research under one roof. CJA utilizes procedural justice practices to provide pretrial services to over 100,000 persons arrested annually in New York City. Information collected about these arrestees and subsequent case processing is maintained in the agency's internal and externally shared database systems, which are used to conduct descriptive and evaluation research on arrestee characteristics, case processing and court outcomes, and on issues and potential reforms to criminal justice policy in New York City. CJA's programs operate in the criminal courts and detention facilities 24 hours a day seven days a week within the 5 boroughs of NYC, employing over 300 employees, citywide.
CJA operates a Supervised Release Program in Queens Criminal Court. The program, which operates during the day and night court in Queens, screens targeted defendants for release at criminal court arraignment, subject to specific conditions including maintaining face-to-face and telephone contact with the program, participating in an assessment process to identify needs such as substance use and mental health, and agreeing to program monitoring of court appearances and re-arrests.
This is a great opportunity for a dynamic, knowledgeable individual, experienced in dealing with high-needs populations such as those with mental health diagnoses, at-risk youth, and others with social, emotional, or developmental challenges, who have criminal court involvement. This position will work with participants aged 16 and older.
Reporting to the Clinical Supervisor ICM and under the direction of the Clinical Director of QSR, the Clinical Case Manager II – Intensive Case Management (ICM) is a dynamic, knowledgeable, and experienced professional with a background in social work, psychology, or behavioral health. This role is ideal for someone passionate about serving individuals with complex needs—including serious mental illness, substance use disorders, housing instability, and trauma—who are also involved in the criminal legal system.
The Clinical Case Manager II will join an innovative, multidisciplinary team that is piloting a supportive and integrated, community-based service delivery mode. This model brings together behavioral health clinicians, peer specialists, housing specialists, and client care coordinators to promote better outcomes for a historically underserved population. The team serves both youth and adult clients (ages 16 and older), providing a unique opportunity to build clinical skills across the lifespan.
This is an exciting opportunity for clinicians seeking to deepen their trauma-informed practice, gain experience in justice system navigation, and earn clinical hours toward licensure. The role offers hands-on experience with evidence-based assessments, care coordination, and advocacy in a real-world setting that values innovation, collaboration, and equity.
Innovative Practice: Be part of a cutting-edge pilot program that integrates clinical, peer, and housing supports to improve outcomes for justice-involved individuals.
Clinical Growth: Gain valuable experience in trauma-informed care, harm reduction, motivational interviewing, and systems navigation—while earning clinical hours toward LCSW licensure.
Meaningful Impact: Work with a population that is often overlooked and underserved, and help shape a model that could influence future service delivery citywide.
Supportive Environment: Join a team that values collaboration, learning, and wellness, with regular opportunities for training and professional development.
The Clinical Case Manager II ICM functions include:
- Conduct comprehensive intake and needs assessments using evidence-based tools to identify mental health, substance use, housing, and other psychosocial needs.
- Provide trauma-informed, person-centered clinical case management to a caseload of high-risk, high-needs youth and adults.
- Collaborate with a multidisciplinary team to develop and implement individualized service plans that promote stability, recovery, and court compliance.
- Support participants in navigating complex systems, including behavioral health, housing, public benefits, and legal services.
- Maintain regular contact with participants through in-person and phone check-ins; monitor progress, re-arrests, and compliance with court mandates.
- Create safety, provide supportive counseling, and encourage client growth and readiness for change.
- Prepare and present written progress reports to the court; advocate for participants' needs and strengths in legal settings.
- Coordinate with defense attorneys, service providers, and community partners to ensure continuity of care.
- Maintain accurate and timely documentation in the agency's case management system.
- Participate in ongoing training, reflective supervision, and team meetings to support professional growth and service quality.
