Case Manager LVN-LPN

HumanGood community is seeking a Case Manager LVN-LPN to support older adults in our senior living community. In this role, you will complete resident assessments, develop and update care plans, and coordinate services to promote safety, independence, and quality of life. You will administer medications and treatments, monitor changes in condition, and collaborate closely with physicians, therapists, and care teams. Working in our mission-driven, resident-focused culture, you will provide compassionate, respectful care while documenting in EHR systems and supporting regulatory compliance.
