Licensed Practical Nurse - ACT Team Location: New York, NY (Onsite) Employment Type: Contract, 20 hours per week Pay Range: $30.87-$44.10 Per Hr Description: Provide nursing services as a member of a mobile multi-disciplinary treatment team. Support individuals with mental illness in securing permanent housing and residential support. Offer psychiatric treatment and individual/group substance abuse treatments. Conduct daily living skills training and financial management skills. Key Responsibilities Conduct physical health assessments. Assess health needs and refer participants to community-based medical care. Manage and administer intramuscular injections and supply medications as prescribed. Provide treatment, rehabilitation, and support services. Meet regularly with participants to assess health status. Provide health management education. Communicate with primary medical providers to coordinate treatment. Qualifications Licensed LPN within the state of New York. Excellent verbal, written, and interpersonal communication skills. Proficient in generic word processing and spreadsheet software. Ability to research material via the internet.
