The work setting fosters a personal touch and a holistic view of patient care, making it critical for you to engage with patients in comfortable, familiar surroundings that they can control.Key ResponsibilitiesIn this role, your responsibilities will include:Directing and managing all aspects of physical therapy treatment, focused on preventing disability, alleviating pain, restoring muscle function, and maximizing performance in alignment with each patient's care plan.Overseeing Physical Therapy Assistants to ensure effective implementation of treatment plans and patient education.Conducting thorough evaluations of clients' physical abilities using diagnostic assessments to devise customized therapy strategies.Assessing clients' home environments and making tailored recommendations to facilitate independence and enhance rehabilitation potential.Providing physical therapy services in client homes, in strict adherence to physician guidelines.Implementing therapeutic exercises that are appropriate for each patient, motivating and instructing them throughout the process.Administering various treatments, including traction, massage, and application of physical agents to assist recovery.Evaluating, fitting, and adjusting prosthetic and orthotic devices, advising necessary modifications to healthcare professionals as needed.Tracking and documenting changes in patient conditions and reporting these observations to physicians and case managers.Participating actively in interdisciplinary team meetings to collaboratively develop and refine comprehensive treatment plans.Preparing and maintaining clinical records such as initial evaluations, progress notes, care plans, and discharge summaries to ensure accurate documentation of each client's status.Educating clients and their families on best practices for utilizing physical therapy devices and adhering to care plans.Completing all orders and documentation punctually, adhering to organizational policies and compliance standards.Performing regular on-site functional assessments and adjusting care plans to optimize the utilization of community healthcare resources on behalf of the client.Patient Population / Client FocusYour primary patient demographic will consist of individuals recovering from orthopedic surgeries, sports injuries, age-related musculoskeletal disorders, and neurological conditions. This demographic requires you to employ a thorough understanding of orthopedic and rehabilitative therapy techniques. You will engage directly with patients and their families, ensuring that all instruction and therapeutic intervention are patient-centric and based on the most up-to-date treatment protocols.QualificationsTo be successful in this role, candidates must possess:A Doctorate of Physical Therapy (DPT) from an accredited institution.A current and unrestricted state license to practice as a Doctor of Physical Therapy.Strong critical thinking and problem-solving skills, capable of making challenging decisions and taking responsibility for them.A collaborative spirit with a commitment to fostering partnerships with clients and team members.Preferred ExperienceAn ideal candidate will have at least one year of experience as a physical therapist in either a home care or acute care setting, showcasing adaptability and familiarity with diverse patient needs.Compensation and BenefitsThe annual salary for this position ranges between USD 100,000 and 120,000, commensurate with experience and qualifications.
