Champion Care is seeking an experienced and relationship-driven Hospital Liaison to support our growing network of skilled nursing and post-acute care facilities throughout the Birmingham market.

The Hospital Liaison serves as the primary marketing and business development representative for the Birmingham market. This role is responsible for developing referral relationships, increasing admissions, and promoting Champion Care's skilled nursing and rehabilitation services to hospitals, physicians, case managers, discharge planners, and other healthcare professionals.

• Build and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, and other referral partners.
• Conduct daily hospital rounds and referral source visits throughout the Birmingham market.
• Educate healthcare professionals on Champion Care's skilled nursing, short-term rehabilitation, long-term care, respite care, and post-acute services.
• Review referrals and coordinate admissions with facility leadership and admissions teams.
• Develop strategies to increase referrals, admissions, and overall census growth.
• Identify new business development opportunities and cultivate referral partnerships.
• Track referral activity, admissions, and market trends to achieve performance goals.
• Represent Champion Care at networking events, healthcare meetings, and community outreach activities.
• Serve as a knowledgeable resource regarding patient placement, insurance considerations, and post-acute care options.
• Maintain strong communication with facility leadership to ensure exceptional customer service and referral satisfaction.

• Previous experience in healthcare sales, hospital liaison, physician relations, admissions, business development, case management, discharge planning, or healthcare marketing preferred.
• Skilled nursing facility (SNF), rehabilitation, long-term care, hospital, home health, hospice, or post-acute care experience strongly preferred.
• Proven ability to build and maintain professional relationships.
• Excellent communication, presentation, negotiation, and organizational skills.
• Self-motivated with the ability to work independently and achieve goals.
• Valid driver's license with reliable transportation.
• Ability to travel throughout the Birmingham market.
