Position Description

• Conduct comprehensive evaluations in both inpatient and outpatient settings and create personalized treatment protocols
• Deliver physical therapy practices that enhance mobility, strength, coordination, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize recovery outcomes
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercise routines
• Document assessment results, therapies, and patient progress as per facility and regulation standards
• Engage in care team discussions and interdisciplinary meetings to promote cohesive care delivery

Qualifications

• Doctorate in Physical Therapy (DPT) or an equivalent doctoral degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (neurological, geriatric) are advantageous but not mandatory

Attributes

• Prior experience is highly preferred
• Capability to treat complex medical conditions and mobility issues
• Excellent communication and collaborative skills
• Familiarity with digital charting and clinical documentation systems
