Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting tailored treatment plans
• Implement therapeutic interventions aimed at improving mobility, strength, balance, and functional abilities
• Collaborate seamlessly with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Guide patients and their families through therapeutic goals, discharge processes, and home exercise programs
• Maintain precise documentation of evaluations, treatment protocols, and patient progress in keeping with facility and regulatory standards
• Engage actively in interdisciplinary care team meetings to assure holistic and coordinated care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
