Hospice Admission Nurse

RTO after booking is not allowed and cannot be worked out onsite. Any call-offs or additional RTO on unit could possibly turn into a cancel and DNR.

Required Skills/Certs/Exp:

Minimum years of experience: 3 years of experience required... Minimum 3 years of experience in home care or hospice. Traveler experience: Unit will accept first-time travelers. Locals accepted/rate difference: Yes. Work setting skills required: Home health/hospice & skilled nursing. Professional knowledge/skills required: Intake assessment/interview & patient/family education and support; setting up DME, medications and supplies for new patients. Additional skills required: Strong writing, organizational & analytical skills. Certs: BLS, RN licensure for the appropriate state, NC.

Preferred skills/certs/exp:

Work setting skills: Inpatient hospice; strong understanding of hospice admission LCD criteria & Epic Dorothy/Comfort experience highly preferred. End of life care skills: Cultural awareness.

Job Details:

Ratios: 2-3 admissions per day, depending on a 10-12-hour schedule. Shift: 40 hours can be offered. Will accept 13 weeks. 10-hour days; 08:00 - 18:30. Call requirement: None. Float requirement: Yes, between the hospital and the field. Weekend requirement: None. Patient diagnoses: These are outpatient hospice admission nurse positions (field and hospital required).

Special Procedures/Unit Details:

Completes the initial clinical assessment, reviews clinical history, current diagnoses and presents a clinical summary to the Hospice Medical Director for hospice. Assesses immediate care needs of the admitted patient and their loved ones, initiates the plan of care and sends associated orders to the Hospice Attending provider. Clinical assessment is completed through head to toe eval, interviewing the patient and loved ones, obtaining a medical history through clinical chart review. Consults with the physician for appropriate orders and assists in obtaining needed supplies, equipment, medications and/or other available services. Evaluates hospitalized patients admitted for acute care against regulatory criteria and facilitates discharge planning. Enters clear, concise, and timely clinical documentation related to the patient's initial Hospice admission and assessment.

Special Equipment: Motor vehicle. Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.

Unit Systems: EPIC, SBAR. Scheduling type: 10 hour days; 08:00 - 18:30. Unit orientation: Approx. 80 hours. Orientation would be approximately 2-3 weeks, as we need them to come with a fundamental hospice experience. Scrubs: Not provided. Navy.
