Job Description Job Description Job Summary: We are seeking a compassionate and motivated Home Care Liaison / Care Coordinator to join our team. This role is responsible for building strong relationships with healthcare providers and community partners while coordinating quality care services for clients receiving home care. The ideal candidate is organized, professional, and passionate about helping individuals receive the care they need in the comfort of their homes. Key Responsibilities: Liaison & Community Outreach Build and maintain relationships with hospitals, physicians, rehabilitation centers, assisted living facilities, and community organizations. Promote the agency’s home care services and generate referrals. Serve as the primary contact for referral sources and new client inquiries. Educate patients, families, and healthcare professionals about available home care services. Care Coordination Coordinate care services for new and existing clients. Work closely with caregivers and families to ensure client needs are met. Assist with client intake, service planning, and care coordination. Monitor client satisfaction and address any concerns promptly. Maintain accurate documentation of client services and communication. Qualifications: Experience in home care, healthcare coordination, case management, or healthcare marketing preferred. Strong communication and relationship-building skills. Excellent organizational and problem-solving abilities. Ability to manage multiple tasks and work independently. Knowledge of home care services and healthcare systems is a plus. Valid driver’s license and reliable transportation.
