Work Environment

This position is full-time and primarily onsite, facilitating home visits to meet patients where they are most comfortable. You will collaborate closely with a dynamic interdisciplinary team, including doctors, nurses, and social workers, to deliver comprehensive and coordinated patient care. Your role will involve assessing patient needs and developing effective treatment plans while utilizing evidence-based practices in home care settings.

Key Responsibilities

• Conduct thorough assessments of patient needs through careful chart reviews and data collection to understand their medical history and physical condition.
• Engage in collaborative discussions with medical professionals and diagnostic imaging teams to integrate patient history and treatment approaches.
• Develop and implement individualized treatment programs that cater specifically to a varied caseload, including orthopedic, neurological, and pediatric populations.
• Utilize innovative therapeutic techniques and modern equipment to enhance patient treatment experiences and outcomes.
• Regularly monitor clinical outcomes, adjusting treatment plans as needed and addressing outlier cases alongside fellow therapists.

Qualifications

• Possession of an active Florida State licensure as a Physical Therapist.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
