Hillcrest Home Care is looking for an experienced, independent, and patient-focused Med/Surg Registered Nurse (RN) to join our home health team serving patients throughout the Omaha area.

You will provide skilled nursing care in patients’ homes while helping them recover, manage chronic conditions, and remain as safe and independent as possible.

What You’ll Do

As a Home Health Med/Surg RN , you will manage an individualized caseload while providing skilled nursing care and education in the home setting.

You will:

• Provide skilled nursing care to patients in their homes.
• Complete comprehensive nursing assessments and monitor changes in condition.
• Develop and carry out individualized plans of care.
• Provide wound care, catheter management, IV therapy, and other skilled nursing services as appropriate.
• Educate patients and caregivers on disease processes, medications, treatments, safety, and self-management.
• Help patients build the confidence and skills needed to safely manage their care at home.
• Collaborate with physicians, therapists, CNAs, and other interdisciplinary team members.
• Communicate changes in patient condition promptly and coordinate appropriate follow-up.
• Complete accurate and timely clinical documentation.
• Support safe transitions from hospital or facility settings back into the home.
• Help patients reduce avoidable complications and remain as independent as possible.
• Manage your schedule and patient visits efficiently while traveling throughout the Omaha area.
• Follow all applicable clinical, regulatory, infection-control, and safety requirements.

What We’re Looking For

We are looking for an RN who is confident in their clinical skills and comfortable working independently in the field.

The right candidate will be organized, adaptable, and able to make sound clinical decisions without needing someone standing beside them. You should also enjoy building relationships with patients and families and helping them understand how to manage their health safely at home.

Ideal candidates will have:

• Strong Med/Surg nursing experience.
• Strong clinical assessment and critical-thinking skills.
• Confidence working independently in a home setting.
• Excellent communication and patient-education skills.
• Ability to recognize changes in condition and determine appropriate next steps.
• Strong organization and time-management skills.
• Accurate and timely documentation habits.
• Comfort managing a flexible schedule and multiple patient visits.
• Ability to collaborate effectively with physicians and interdisciplinary team members.
• A patient-centered approach focused on safety, independence, and quality of life.

Education & Credentials

• Current Nebraska or compact-state Registered Nurse license required .
• Minimum one year of home health nursing experience required .
• OASIS experience preferred.
• Experience with catheter management, wound care, and IV therapy preferred .
• Valid driver’s license required.
• Reliable transportation and current automobile insurance required.

Physical & Other Requirements

• Must be able to speak, read, and write English.
• Must be able to safely provide hands-on nursing care in a variety of home environments.
• Must be able to perform repetitive sitting, standing, walking, bending, reaching, lifting, and movement associated with patient care.
• Must be able to work independently and adapt to different patient and home environments.
• Must be flexible and calm when responding to changing patient needs.
• Must have strong attention to detail and be able to manage a mobile caseload.
• Must be able to utilize clinical technology and electronic documentation systems efficiently and effectively.
• Must be able to travel throughout the Omaha service area.
• Must maintain a valid driver’s license, reliable transportation, and current automobile insurance.
• Other duties as assigned.
