In this role, you will conduct clinical assessments, create and manage care plans, facilitate transitions of care, and collaborate with providers, members, and community resources to optimize quality, safety, and cost-effective outcomes. You will educate patients and families, monitor progress, and use data and clinical guidelines to reduce hospitalizations.
