Home Health Physical Therapist

Job Location: Goodyear, AZ. Pay: $100,000.00 - $120,000.00 per year, plus Signing Bonus. Immediate Opening: Home Health PT - Southwest Phoenix. Full-Time/Part-time. Job Responsibilities Assess patient’s home environment for safety hazards and architectural barriers, identify equipment needs. Identify functional problems and establish goals and a plan of care designed to improve the patient’s functional status (revising as necessary). Perform patient tests, measurements, and evaluations, such as range of motion, manual muscle tests, gait and functional analysis, body parts measurement, and record and evaluate findings to aid in establishing or revising specifics of treatment programs. Perform and complete start-of-care and ongoing assessments and associated paperwork in accordance with agency policy. Periodically re-evaluate and upgrade the patient’s program to maximize functional improvement. Administer the exercise program utilizing specialized therapeutic exercise techniques and equipment. Plan and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts. Recommend prosthetic/orthotic devices for patients or evaluate their devices and make minor adjustments or recommend modifications by the prosthetist/orthotist. Conferring with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care. Perform and submit required documentation of patient’s evaluations, plan of care, clinical notes, progress reports, summaries along with changes in orders. All patient visit notes must be turned in the next scheduled workday, not to exceed forty-eight (48) hours. Strive to maintain positive internal and external customer service relationships. Demonstrate effective leadership skills. Demonstrate effective communication skills by conveying necessary information accurately, listening effectively, and asking questions when clarification is needed. Be dependable to plan and organize work effectively and ensure completion. Show reliability by arriving to work on time and taking breaks in expected time frames. Meet all productivity requirements. Exhibit team behavior and promote a team‑oriented environment. Represent the organization professionally at all times. Show initiative and continually improve processes and relationships. Education & Experience Graduate of an accredited Physical Therapy program and licensed by the state of Arizona. Previous experience in a clinical setting is preferred; motivated new graduates are welcome. Excellent communication and interpersonal skills.
