Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients .

Compensation:

• Local Pay: $53–$58/hour (W-2)
• Travel Pay: $2,400–$2,550 gross per week
• Mileage Reimbursement: $0.50/mile (from first patient to last patient)

Location: Flagstaff, AZ, 86001

Responsibilities:

• Deliver compassionate hospice home health nursing care by performing comprehensive patient assessments, symptom and pain management, medication administration, care planning, documentation in HCHB (Homecare Homebase) , and coordinating with physicians, caregivers, and the interdisciplinary team to ensure quality end-of-life care.
• Manage a 60-mile service area while meeting 30 productivity points per week , participating in scheduled on-call/weekend rotations , responding promptly to urgent patient needs, maintaining accurate and timely documentation, and adhering to hospice standards, agency policies, and infection control practices.

Weekly Hour Guarantee: 40

Productivity Expectations (points): 30

Coverage Areas: 60 mile radii from agency (Flagstaff, AZ, 86001)

Shift schedule (days, start & end times): 8 AM to 4:30 PM regular; on call 430p-8a Mon – Fri. Weekend Fri 430p-Mon 8a

On Call/Weekend Requirement: Yes

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes ($0.50/Miles)

Must have qualifications/experience: Active AZ RN license or compact, recent HCHB experience, hospice.

Dress code/scrub color: scrubs, any color, clean, neat free from wrinkles,n tears, holes, fray, smell
