About the Role:

• Provide direct patient care to residents in our skilled nursing facility, including administering medications, documenting patient records, and developing individualized care plans.

• Conduct routine assessments and evaluations to identify patient needs and develop effective care strategies.

• Collaborate with registered nurses, doctors, and other healthcare professionals to ensure seamless resident care.

• Maintain accurate and up-to-date records, reports, and charts.

• Demonstrate a commitment to patient-centered care, upholding the highest standards of quality, confidentiality, and respect.

Responsibilities:

• Provide skilled nursing care to residents, including wound care, medication administration, and respiratory therapy.

• Develop and implement individualized care plans in collaboration with the multidisciplinary team.

• Participate in quality improvement initiatives and contribute to a culture of excellence.

• Maintain a clean and safe environment, adhering to facility policies and procedures.

• Communicate effectively with patients, families, and healthcare team members.

• Participate in educational and training programs to enhance knowledge and skills.
