Certification Requirements CA RN License BLS Certification Experience 5 years of RN Case Management experience Inpatient Acute Case Management Job Duties Coordinate care and transitions throughout the acute care patient experience. Collaborate with physicians, medical social workers, and bedside RNs. Ensure timely progression and transition of patients to prevent unnecessary admissions or readmissions. Communicate effectively and facilitate care across the continuum. Develop cost-effective care coordination plans. Identify and refer quality and risk management concerns. Job Accountabilities Conduct patient assessments and develop care plans. Manage utilization and ensure appropriate level of care. Formulate and implement transition plans. Address complex clinical and social situations to avoid unnecessary admissions. Participate in department operations and improve practices.
