Physician (MD/DO) - In-home Visits

Role Physician (MD/DO) contractor conducting in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members. Responsibilities Perform in-home visits focusing on overall health, well-being, and identifying care barriers. Review medication and medical history, conduct physical evaluations without prescribing or altering treatment. Complete pre-populated, patient-specific assessment forms. Schedule and manage visits independently, ensuring comprehensive assessments. Qualifications & Skills State medical license required; willingness to enroll in Medicaid preferred. Ability to work flexible hours, with a preference for availability across weekdays and weekends. Strong communication skills and ability to connect with patients one‑on‑one. Independent contractor status (1099), with malpractice coverage provided.
