The Community Health Specialist provides assistance to maintain the necessary data elements of the community health improvement programs, including but not limited to tracking metrics, preparing data, collecting and tracking trends and serving as a liaison within the community when necessary.

WHAT YOU CAN EXPECT

• Schedule: Monday - Friday, 8am - 4:30pm (some weekend and evening availability will be needed for community events)
  
• Support the Prenatal Assistance Program by acting as bridge between health care providers, community services providers, and individuals in the community to promote health, reduce disparities, and improve outcomes for mothers, babies, and families.
  
  
• Organize and participate in special events related to community health improvement and community benefit internally and in the community.
  
  
• Attend appropriate conferences and meetings as the hospital's representative and share findings with leadership as necessary.
  
  
• Support community initiatives to promote collaborative arrangements that meet the health needs of the community.
  
  
• Coordinate creation of community benefit reports and ensures accurate reporting for the state government, federal government, and local community benefit reporting.
  
  
• Provide support and assist coworkers across various departments as needed, and completes duties as assigned by the management staff.
  
  
• Maintain guidelines for quality assurance programs.
  
  
• Monitor and maintain productivity and statistical records related to program operations.
  
  

QUALIFICATIONS

• Bachelor's degree in Healthcare Administration(Business), Public Health, or Public Relations required
  
  
• 10 years of Community Health experience required in lieu of Bachelor's degree
  
  
• 1 year of Healthcare or Non-Profit experience required
