Registered Nurse (RN) - Pediatric & Adult Home Health | Private Duty Nursing

What You’ll Do Care for medically complex patients, primarily children, in the comfort of their homes Provide personalized nursing care for pediatric and/or adult patients with complex needs Coordinate care plans with physicians, therapists, and other team members Monitor patient progress and update care plans as needed Educate and support families with in‑home care techniques Document visits and care notes in the electronic charting system (EMR) Collaborate with clinical supervisor and participate in case conferences Act as an advocate for patient welfare and coordinate care between patients, families, and other providers Develop, deliver, and revise individualized patient‑centered care plans under physician supervision Administer medication per agency policy and physician orders Maintain medication reconciliation and report changes in patient condition Perform initial evaluation visits and reevaluations of patient nursing needs Prepare clinical and progress notes for each patient visit Assist with preventive and rehabilitative nursing procedures Guarantee confidentiality of patient, employee, and agency matters Participate in quality assurance and performance improvement activities Take on‑call duty nights, weekends, and holidays as assigned What You Bring Active RN license and CPR certification Clinical experience preferred; new graduate candidates welcome Excellent communication and critical thinking skills A love for patient‑centered care, especially in home environments Confidence, compassion, and a calm presence during routine care and critical moments Strong verbal and written communication skills Ability to maintain licensure and completing continuing education as required Physical ability to perform the described duties and secure transportation Ability to organize work procedures through time management and supervise ancillary disciplines Benefits Medical, Dental, and Vision Insurance 401(k) with company match Referral Program Flexible scheduling Paid, hands‑on training Weekly pay Tuition Assistance Programs and Continuing Education opportunities Professional development for new graduates Competitive daily and weekly pay options ($25.00 - $35.00 per hour) Team Select Home Care reserves the right to change the above job description and qualifications without notice.
