Day Services Coordinator

We are currently hiring for a Day Services Coordinator. Our program coordinators assist consumers with intellectual and developmental disabilities in our day program. Coordinators complete comprehensive assessments, individual support plans and daily service plan follow up. Coordinators also implement consumer treatment plan to enable maximum independence, involvement, and success in the community. Available Position Full‑time; 40 hours per week Monday‑Friday 7:30 am to 3:30 pm with some Saturdays as needed Wage: From $18.75 per hour Hiring Bonus: $1,000 (paid in one installment upon completion of 6‑month continued employment) As the Day Services Coordinator, you will play a crucial role in creating a supportive and inclusive environment. You will collaborate with a team of dedicated professionals to develop and implement engaging activities, ensuring that each participant's needs and interests are met. Additionally, you will oversee the day‑to‑day operations of the program, ensuring a smooth and effective program delivery. Duties and Responsibilities Direct Services Coordinators address the wants and needs of people with intellectual and developmental disabilities, which include: Promote and encourage community integration and socialization opportunities in a positive manner. Coordinate daily activities for participants in the day program. Multitasking and excellent time management skills. Provide individualized support to participants, taking into account their unique needs and goals. Collaborate with other team members to design and implement individualized care plans. Monitor the progress of participants and make necessary adjustments to their care plans. Reinforce therapies, including speech, behavioral support and physical therapy. Administer medications and maintain accurate medication records. Maintain accurate and up‑to‑date documentation of participant progress and program activities. Ensure all needs are addressed in a respectful manner with dignity and personal choice. Complete and maintain required documentation and trainings.
