Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, direct observations, and meticulous reviews of medical records, carefully evaluating the emotional, social, and environmental factors that can influence patient recovery.

• Deliver empathetic counseling and emotional support to patients and their families, facilitating discussions around adjustment issues, grief, and coping mechanisms associated with recovery.

• Collaborate closely with an interdisciplinary rehabilitation team to develop personalized treatment plans that truly reflect each patient's psychosocial needs and rehabilitation goals.

• Guide patients in navigating community resources, including support groups, vocational rehabilitation options, and housing assistance, to ensure a smooth transition back to daily life.

• Provide educational support to patients and their families regarding accessing post-rehabilitation services and community resources, equipping them with tools for successful reintegration into the community.

• Participate actively in discharge planning processes to ensure patients transition efficiently from inpatient care to their home environment or other care settings.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 75,000 to 100,000 annually, commensurate with experience and qualifications, along with potential sign-on bonuses.
