Key Responsibilities\n

• Provide counseling and emotional support to patients and their families, focusing on adjustment issues related to illness or injury, grief, and emotional well-being.

• Collaborate with rehabilitation team members, including physical therapists and occupational therapists, to develop tailored treatment plans considering the psychosocial needs of each patient.

• Assist patients in accessing various community resources such as support groups, vocational rehabilitation programs, and housing assistance to enhance their recovery journey.

• Educate patients and their families on available community resources and post-rehabilitation services to ensure smooth transitions post-discharge.

• Actively participate in discharge planning, ensuring patients are prepared for successful reintegration into their homes or other care environments.
