Registered Nurse (Rn) - Specialty Care Nurse Coordinator

The Registered Nurse (RN) - Specialty Care Nurse Coordinator delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. The RN is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal specialty care. The focus is on individual growth and development in practice with increased levels of self-direction. This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Ability to formulate a plan of care based on nursing assessments and Veteran goals of care. Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. The RN brings principles and theory with limited application of critical thinking skills. Additional duties as assigned.

Work Schedule: Monday - Friday 7:30 a.m. - 4:00 p.m.

Preferred Experience: Recent experience (within the last 5 years):

• Outpatient Wound Care Management: Experience in the assessment and management of acute and chronic wounds in an outpatient setting, including treatment planning, dressing selection, and evaluation of healing outcomes.
• Wound Care certification
• Dermatology and/or Procedural Clinic Experience: Experience supporting dermatologic or surgical procedures, with knowledge of skin conditions, biopsies, excisions, and post-procedure care.
• Procedural Coordination (e.g., Mohs Surgery): Experience coordinating surgical procedures, including patient scheduling, pre-procedure preparation, patient education, and post-procedure follow-up.
• Patient and Caregiver Education: Demonstrated ability to provide clear, effective education on wound care, post-procedural care, and infection prevention to support optimal outcomes.
• Interdisciplinary Collaboration: Experience working with providers, nursing staff, and ancillary services to coordinate care, manage consults, and ensure continuity across settings.
• Clinical Documentation and Workload Management: Proficiency in electronic health record documentation (e.g., CPRS) and ability to manage workload in a high-volume outpatient clinic while maintaining timely, accurate documentation.
