Key Responsibilities\n

• Conduct comprehensive psychosocial assessments by interviewing patients, observing behaviors, and reviewing medical histories to identify emotional, social, and environmental factors affecting their recovery.

• Provide expert counseling sessions for patients and their families, focusing on issues of adjustment, coping strategies, and emotional support during the recovery process.

• Collaborate with a multidisciplinary rehabilitation team to create individualized treatment plans that incorporate the psychosocial needs of patients, ensuring a holistic approach to their care.

• Facilitate patient access to community resources such as support groups, vocational rehabilitation services, and housing assistance, vital for their long-term recovery.

• Educate patients and families about post-rehabilitation services and community resources available to support their well-being after discharge.

• Engage in discharge planning by coordinating with team members to ensure seamless transitions to home and community-based care.

You will work primarily with a diverse range of patients undergoing rehabilitation, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n
