Key Responsibilities\n

• Conduct comprehensive assessments for clients entering the detox program, evaluating their psychosocial needs and formulating actionable care plans.

• Provide individual and family counseling sessions, focusing on coping strategies, emotional support, and relapse prevention techniques.

• Collaborate with an interdisciplinary team of healthcare providers, including nurses, addiction specialists, and counselors, to coordinate comprehensive care.

• Advocate for client needs by connecting them with necessary community resources, ensuring their emotional and practical support throughout the recovery process.

• Maintain thorough and accurate documentation, including assessments, care plans, progress notes, and client interactions in electronic health record (EHR) systems.

• Monitor client progress through regular virtual check-ins, adjusting care plans as necessary based on evolving needs and circumstances.

• Provide crisis intervention when required, offering immediate support and resources to clients and their families during difficult situations.

• Facilitate support groups and workshops to educate clients on recovery processes and available support options.

Qualifications\n
