Job Description

Medical Director - Coordinated Care (Family Medicine)

Build Programs, Not Burnout - In the Land of 10,000 Lakes

Position Highlights:

Work-Life Balance: Flexible schedule with a 3-5 day work week and just one day per week in a structured correctional care setting. No call, no holidays, or weekend requirements.
Desirable Practice Setting: Modern, patient-centered clinic with integrated care delivery
Ideal Work Environment: 10–12 patients per day supported by experienced APP staff
Leadership Opportunity: Lead and grow a large health system’s coordinated care program with meaningful impact on patient outcomes
Culture & Support: Collaborative team environment with strong clinical and administrative backing

Compensation & Benefits:

Competitive Compensation: Strong base salary depending on experience
Generous Quality Bonus: Incentives tied to quality outcomes and program success
Sign-On Bonus: $100,000
Relocation Stipend: Up to $15,000 based on location
Student Loan Repayment Assistance: System-funded and paid directly to lender

Qualifications:

Degree: MD/DO
Certifications: Board certification in Family Medicine, Addiction Medicine, Internal Medicine, or IM/PEDs
Licensure: MN license or ability to obtain
Skillset: Interest in or comfort Addiction Medicine and underserved populations
Experience: Open to residents and fellows, leadership interest encouraged
