Visiting Nurse Health System is looking for an experienced and mission-driven Director of Case Management - EDWP to lead care coordination for our Elderly and Disabled Waiver Program. In this highly visible leadership role, you will provide strategic leadership, operational oversight, and clinical guidance for case management services in VNHS community-based programs. This in-office position is based at our Hartsfield or Marietta office, Monday through Friday.

Job Overview:

The Director of Case Management (DCM) for Visiting Nurse Health System’s (VNHS) Elderly and Disabled Waiver Program (EDWP) ensures high-quality, member-centered care coordination for older adults and individuals with disabilities who have medical and social needs. Serving as a liaison to the Executive Director of Operations (EDO), the DCM is a key member of the leadership team, aligning case management operations with organizational goals and promoting equitable access to care. The DCM leads a large team, including the Sr. Manager of Case Management, Team Leads, and clinical and non-clinical staff across multiple locations.

The DCM develops and implements case management models to promote service access, improve health and social outcomes, and ensure compliance with regulatory, contractual, and funding requirements. The DCM oversees multidisciplinary teams, sets performance standards, and builds partnerships with healthcare providers, social service agencies, government entities, and community organizations.

What you'll be doing:

The Director of Case Management - EDWP provides strategic leadership and oversight of case management services across community programs, ensuring high-quality, member-centered care, regulatory compliance, and strong operational performance. This role leads clinical and non-clinical teams, drives quality improvement initiatives, develops community partnerships, and ensures effective coordination of services.

• Lead and oversee EDWP case management operations, policies, quality assurance, and performance improvement initiatives. 
• Provide leadership, coaching, and direction to the Sr. Manager of Case Management, Team Leads, Case Managers, and support staff. 
• Monitor performance metrics, clinical outcomes, compliance, and productivity; identify trends and implement corrective action plans. 
• Develop strategic initiatives and community partnerships that strengthen care coordination, resource integration, and program outcomes. 
• Provide clinical consultation, participate in assessment conferences, conduct joint home visits when necessary, and review documentation for quality and compliance. 
• Oversee recruitment, staffing, onboarding, performance management, training, and professional development for case management teams. 
• Ensure services are delivered in accordance with EDWP, Medicaid, Medicare, accreditation, reimbursement, and VNHS standards . 
• Serve as the primary liaison with the Department of Community Health (DCH) for the EDWP. 
• Lead complaint resolution, clinical investigations, risk management, and regulatory compliance activities. 
• Foster a collaborative, culturally responsive, and member-centered environment through effective communication and team engagement. 
• Promote member safety, infection control, and evidence-based practices throughout service delivery.

Minimum Qualifications:

• Bachelor’s degree in human services, health sciences, social work, or a related field.
• Master’s degree is a plus (MSW or MHA). 
• Licensed Clinical Social Worker is a plus (LCSW).
• Licensed Master Social Worker is a plus (LMSW).
• Certified Social Work Case Manager (C-SWCM) is a plus.
• Valid CPR certification.
• Current, valid driver’s license.
• Minimum of five years of case management experience.
• Minimum of five years of leadership experience.
• Minimum of five years of experience working with Medicaid Waiver programs.
• Minimum of five years of experience in community-based healthcare, public health, care coordination, or a similar healthcare or social services role.
• Knowledge of Medicaid Waiver policies and regulations.
• Demonstrated knowledge of KPIs and performance management.
• Strong knowledge of healthcare regulations, quality standards, and care coordination models.
• Strong understanding of community health issues and social determinants of health and access to care.
• Demonstrated proficiency using digital tools and platforms for communication, documentation, reporting, and data tracking.

• Experience working with underserved or vulnerable populations.

• Proficiency with Microsoft Office applications, internet navigation, email, computer file management, online HR services, and online training platforms. Knowledge of the Georgia Horizon system is a plus.

Salary Range: $90,000.00- $115,000.00
