Licensed Practical Nurse (LPN) Location: 119-Southern Utah Veterans Home - Ivins, St. George, UT, 84738 Position Summary The Licensed Practical Nurse (LPN) provides direct nursing care to residents at the Southern Utah Veterans Home. The LPN works under the supervision of a Registered Nurse (RN) and collaborates with the healthcare team to ensure high-quality, compassionate care for our veteran residents. Key Responsibilities Administer medications and treatments as prescribed by physicians and according to facility policies. Monitor and document residents’ health status, including vital signs, symptoms, and changes in condition. Assist with the development and implementation of individualized care plans. Provide wound care, catheter care, and other nursing procedures as required. Collaborate with RNs, physicians, and other healthcare professionals to coordinate resident care. Maintain accurate and timely documentation in residents’ medical records. Respond promptly to resident needs and emergencies. Educate residents and their families on health maintenance and disease prevention. Adhere to all facility policies, procedures, and regulatory requirements. Qualifications Current and valid Licensed Practical Nurse (LPN) license in the state of Utah. Minimum of 1 year nursing experience preferred, preferably in long-term care or geriatric settings. Basic Life Support (BLS) certification required. Strong communication and interpersonal skills. Ability to work effectively both independently and as part of a multidisciplinary team. Compassionate and patient-centered approach to care. Work Environment The Southern Utah Veterans Home is dedicated to providing a supportive and respectful environment for veterans.
