The Department of Health and Social Services (DHSS) seeks a Health/Human Service Case Manager II to determine eligibility for needs-based benefits including SNAP, Medical Assistance, and TANF. You will work with TANF and SNAP vendors, provide case management, and refer clients to housing, employment, and food resources. You will operate within a multidisciplinary team using trauma-informed approaches to identify barriers and connect low-income residents with benefits while maintaining #J-18808-Ljbffr DelDOT
