Schedule Night Shifts - 12 hour shifts. Taking call is required 2 shifts per 6 weeks with potential for overtime. Job Description The LPN providing care to the patient population(s) is responsible for assessing, planning, implementing, and coordinating patient care in Skilled Nursing/Extended Care settings. ResponsibilitiesAssess and evaluate patient needs and care requirements. Plan and implement nursing care as per established clinical guidelines and protocols. Coordinate with healthcare team members to ensure optimal patient care. Administer medications and perform treatments as prescribed. Document patient progress and communicate effectively with the team. Required Experience / Certifications / LicensureCurrent LPN licensure in the state of Colorado.
