Geriatric PA — Home-Based Primary Care
PINE PARK HEALTH MANAGEMENT INC
Pine Park Health is a value-based primary care practice focused on senior living communities. The role delivers on-site primary care for older adults in Tempe/ Phoenix area, working with a multidisciplinary team to implement person-centered care plans. You will support a smaller patient panel of complex cases, contribute to a value-based care model, and travel within the Phoenix area to partner communities.
