Nurse Practitioner / Physician Assistant - Primary Care | Home-Based Care

Location: Casa Grande / Maricopa, Arizona

Salary: $140,000-$150,000 + Bonus

Schedule:

• Monday-Friday
• 8:00 AM-5:00 PM
• No evenings or weekends

Are you an experienced Nurse Practitioner or Physician Assistant looking to make a meaningful impact outside of the traditional clinic setting?

We're seeking a compassionate and experienced Advanced Practice Provider to deliver primary care to medically complex patients in their homes throughout the Casa Grande and Maricopa area.

This role focuses on caring for patients living with chronic kidney disease, end-stage renal disease, and multiple chronic medical conditions through an innovative value-based care model designed to improve outcomes while reducing unnecessary hospitalizations.

What You'll Do

• Provide comprehensive primary care in patients' homes
• Manage chronic and complex medical conditions
• Conduct approximately five patient visits per day
• Complete 80% in-home visits and 20% virtual visits
• Collaborate with an interdisciplinary care team
• Support patients during care transitions following hospital discharge
• Build long-term relationships with a manageable patient panel

Qualifications

• Licensed Nurse Practitioner (preferred) or Physician Assistant
• 3-5+ years of APP experience
• Primary Care, Family Medicine, Internal Medicine, or Geriatrics experience
• Comfortable traveling throughout the assigned territory
• Strong communication, organization, and documentation skills

Compensation & Benefits

• $140,000-$150,000 base salary
• 10% annual performance bonus
• $10,000 retention bonus
• $3,500 relocation assistance
