Department: Quality Management Under the supervision of the Director of Quality and Patient Safety, the Disease-Specific Care Coordinator functions as an expert nurse in the disease specific field, resource for nursing staff and patient care in this area, consultant, researcher and outcomes focused specialist. The Coordinator is responsible for providing coordination and oversight for the assigned hospital’s disease-specific program. This multidimensional role will focus on current treatment, prevention, education to improve quality of patient care. The Coordinator will drive current certification through performance improvement activities and standard compliance. Regularly performs tasks to ensure efficient operations of all aspects for the disease-specific program. Serves as a liaison between quality improvement, nursing and medical staff in the disease-specific program at SVH. This position is responsible for reporting and presenting accurate, quality information both internally and externally. Responsible for site compliance: reviews source documentation case report forms, data reports for accuracy, and ensures timely submission of such documentation to assure timely compliance with program requirements. Has primary responsibility for leading, controlling, planning, organizing and evaluating the timely, effective and efficient function of the program data and standards. They collaborate in the implementation of multidisciplinary standards for patient with disease-specific care and promote effective control mechanisms to ensure that the physical environment fosters safe patient care. Collaborates in planning, implementing, and evaluating department activities including appropriate knowledge of PI practice and theory to assure professional competency to ensure care is evidence based. The Coordinator acts as a role model, preceptor of staff, ensures the consistent application of the Hospital’s policies and procedures and fosters teamwork within and between areas of responsibility. The Coordinator is also responsible for promoting and participating in specialized knowledge and skills through the support of staff education and professional development. As a clinical resource to clinical and medical staff in providing in-services, and advising management on training needs. The Coordinator acts as an effective liaison to medical staff to ensure continuity and congruity of hospital services in accordance with the plan of care. Facilitates and coordinates program development in order to maintain Joint Commission Disease-Specific Certification, or accreditation by other certifying agencies (i.e., Society of Cardiovascular Patient Care [SCPC]). Applies the quality improvement process to implement and evaluate quality of care and make recommendations for implementation of new technology or change in disease-specific care, treatment, management and prevention practices. Assists with development and implementation of programs to support the needs of patients and their families and goals of the Disease-Specific Program. Participates in relevant quality and performance improvements. Assists in the development of appropriate educational material for patients, family, and clinical staff in collaboration with Education, Nursing/Clinical and Medical Staff. Initiates and performs ongoing review of policies and protocols related to services provided to ensure standards are followed. When appropriate, updates or writes new policies to enhance professional practice. Promotes and participates in improving patient care and in quality performance improvement programs by taking primary responsibility for data collection and analysis of results to support the nursing departments. Assist in ongoing clinical performance audits in the clinical departments in order to ensure regulatory compliance, promote patient safety and establish quality outcomes. Facilitates collection of, analyzes and presents disease-specific certification program data and research at appropriate physician and clinical meetings. Uses data to set benchmarks for hospital performance improvement programs. Serves as a resource/liaison for staff assigned to assist with disease-specific care research and data collection as needed. Trains data entry and data acquisition personnel as needed. Designs, tests, documents and maintains an audit process for data quality validation. Mentors staff in the understanding of disease-specific care and research in quality improvement for patient population. Assists staff to use evidence-based outcome data to improve patient care. Assures that complete and accurate data is collected, calculated and documented. Maintains integrity of data security, confidentiality and preservation for the control and use of program information. Attends and participates in the committees that relate to the Disease-Specific Care Program. Consistently provides timely accurate reports and updates to the Leadership Team, the Medical Staff committees and the Quality and Safety Committee as appropriate. Communicates any patient-related clinical issues or problems of the department requiring additional assistance to the Director and/or Manager immediately and to the Senior Administrative Director of Regulatory Compliance & Organizational Improvement. Maintains an active role in outcome data management of the Disease-Specific Program. Ensures that certification requirements for data submission are met. Review case files relating to untoward patient outcomes within the disease-specific population being served. Clarifies and determines if the customer’s expectations were met and Reviews committee case referrals in a timely manner to develop effective training programs based on findings. Promotes professional development by collaborating in training and orientation of new employees, unit education, in-services, competency validation and other related activities. Facilitates and conducts effective hospital and community-based training, in-services, symposiums, conferences, physician outreach and meetings on disease-specific treatment and prevention. Functions as a clinical resource for staff and patient/family. Serves as a resource/liaison for hospital clinical staff in understanding and utilizing clinical outcomes data to improve patient care. Collaborates with clinical staff on educational needs in regards to clinical pathways. Stays abreast of definitions and regulatory updates/changes mandated by registries/societies, clinical research and other clinical outcomes programs. Maintains knowledge of public reporting initiatives. Maintains knowledge of state and federal legislation impacting data submission and reporting requirements. Consultant and clinical expert in specialty area and provides liaison relationship between staff, physicians, and other members of the multidisciplinary team. Collaborates with Physicians, hospital staff, Medical Staff Office and Regulatory and Education personnel. Collaborates with physicians and other hospital personnel in developing and maintaining a comprehensive disease-specific care certification program. Reviews operational information and ensures systems are in place to monitor, review and revise procedures in order to maximize resource utilization within the program. Working closely with the Medical Director of the Disease-Specific Service, will develop and maintain an ongoing disease-specific certification database. Develops community and professional referral system and outreach programs. Oversees community education events in conjunction with organizations such as, but not limited to: National Stroke Association, American Stroke Association and American Diabetes Association. Organizes and participates in stroke risk prevention screenings, as well as other disease-specific programs. Utilizes positive communication skills to affect resolution and teamwork. Performs other duties as assigned. Job Requirements: Education: Bachelor’s Degree in Nursing preferred. Licensure: Licensed as a Registered Nurse in the State of California required. Clinical Nurse Specialist/Nurse Practitioner certification preferred. Experience: Previous program development and implementation experience preferred with a minimum of four (4) years of clinical nursing experience. Recent experience in acute clinical neurosciences preferred. Knowledge of Institutional Review Board regulations and experience with screening and recruitment of patients into clinical trials preferred. Demonstrates skills and knowledge in management theories, practices and techniques. Comprehensive knowledge of clinical pathway development. Demonstrated teaching experience in acute care and community settings. Complete understanding of acuity levels for both inpatient and outpatient populations. Current knowledge of hospital regulations, patient rights and consent law. Thorough knowledge of TJC Disease-Specific and other regulatory requirements. Pay Range: The hourly rate for this position is $72.45 - $90.00. The range displayed on this job posting reflects the target for new hire salaries for this position.
