Case Manager

Hospice of the East Bay (HEB) is a community-based nonprofit hospice and palliative care organization that has been serving patients and families with compassion and dignity for nearly 50 years. We provide hospice and palliative care in patients' homes and in our six-bed inpatient hospice facility, ensuring every patient receives exceptional care wherever they are most comfortable. Our interdisciplinary team works together to support not only our patients but also their loved ones, offering bereavement services for a minimum of one year after a loss. This position serves patients in Vallejo, Hercules, Richmond, Emeryville, Oakland, and surrounding areas . Position Summary Reporting to the Team Manager, the RN Case Manager plays a key role in assessing and managing the physical, emotional, social, and spiritual needs of our patients and their families. You will coordinate and implement personalized care plans in partnership with the attending physician, Medical Director, and a dedicated interdisciplinary team. Each RN Case Manager is responsible for a manageable caseload of up to 15 patients and is supported by Revisit Nurses and the Team Manager. Key Responsibilities Recognize and respond to signs of active dying and patient transition Effectively assess and manage pain and symptoms based on individualized care plans Educate patients, families, and caregivers using clear and compassionate communication Document all assessments, interventions, and patient progress accurately and promptly Deliver holistic, patient- and family-centered care across diverse care settings Honor patients' cultural and spiritual beliefs throughout their journey Collaborate with interdisciplinary team members and external providers to ensure quality care Coordinate care with attention to insurance, authorizations, and cost considerations Facilitate care transitions such as General Inpatient (GIP) or Respite when needed Supervise LVNs and Home Health Aides with clarity and support Participate in team meetings, training sessions, and quality improvement activities Maintain compliance with all applicable state, federal, and accreditation standards Who You Are A compassionate RN with strong clinical skills and a desire to support patients and families at a deeply meaningful time in their lives Able to perform comprehensive assessments and create thoughtful, individualized care plans Emotionally resilient, with the ability to navigate patient/family stress and grief with empathy and professionalism A strong communicator who values teamwork, but can also work independently and manage crises effectively Qualifications Current California RN license Minimum of one year of acute care nursing experience CPR certification Valid California driver's license and reliable transportation Experience in hospice or home health (preferred) Certification in Hospice and Palliative Care Nursing (highly desirable) IV therapy experience (a plus) Proficient in basic computer applications and electronic health records Compensation & Benefits $2.00/hour additional pay for this service area 403(b) retirement plan with generous company match Medical, dental, and vision coverage , with HSA-eligible plans available Up to 27 days of PTO and paid holidays Mileage reimbursement at the IRS rate Tuition reimbursement Life insurance Pet insurance and employee discount programs Employee Assistance Program (EAP) A collaborative, mission-driven team that truly cares Together, we provide more than care; we offer peace of mind, dignity, and heart.
