Responsibilities

• Provide direct patient care to home health patients, including assessment and treatment planning

• Deliver evidence-based interventions, document progress, and adjust plans as needed
• Collaborate with the care team and communicate goals and progress to patients and families
• Maintain timely, accurate documentation and ensure compliance with applicable standards

Requirements

• Active Physical Therapist license in Montana or eligibility for licensure
• Demonstrated ability to perform direct patient care in a home health or hospital-side setting
• Strong communication and documentation skills, with the ability to work with a multidisciplinary team
• Willingness to commit to a 16-week travel assignment and the 4x10-hour shift pattern
