Key Responsibilities\n

• Conduct thorough and culturally sensitive biopsychosocial assessments tailored to the mental health, substance abuse, and medical needs of patients within the primary care context.

• Deliver a range of behavioral health services including individual, couples, and family counseling, comprehensive assessments, crisis intervention, and treatment planning.
\n
• Maintain meticulous documentation of mental health evaluations, progress notes, treatment plans, and suicide risk assessments in our electronic health record system.
\n
• Collaborate closely with medical providers and your clinical team to ensure appropriate referral and escalation of patient needs.
\n
• Facilitate access to community resources and support services, such as medication assistance programs, to enhance patient well-being and recovery.
\n
• Engage in continuous quality improvement efforts by collaborating with our Quality Improvement teams to develop processes for effective data collection and analysis.

Patient Population / Client Focus\n

In this role, you will work primarily with a diverse range of patients from various cultural and socioeconomic backgrounds. Our patient population includes individuals struggling with mental health disorders, substance abuse issues, and those in need of psychosocial support. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A valid and active Licensed Clinical Social Worker (LCSW) license in Pennsylvania.

• A minimum of three (3) years of post-licensure experience in a clinical or mental health setting.

• Solid familiarity with crisis intervention strategies and counseling techniques that facilitate effective patient management.

Compensation and Benefits\n
