Licensed Practical Nurse

The Licensed Practical Nurse is responsible for providing nursing services to patients in the home or community settings under the direct supervision of a Registered Nurse. All functions shall be performed in accordance with the established policies and regulations and standards of care for a Licensed Practical Nurse. Essential Functions Provide professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per the Company’s guidelines. Provide skilled nursing visits for patients as directed by the RN Case Manager. Follows a care plan established by the Registered Nurse. Work closely with other members of the interdisciplinary team to enhance care and support for the patient and family. Scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration and monitoring, emergency intervention, and nursing serves within the professional scope and as delegated by the RN Case Manager. Demonstrate knowledge of safety/infection control practices by compliance with policies and procedures. Ensure quality and safe delivery of hospice services to patients, families, and caregivers. Ensure that equipment, supplies, and other necessary items and services are available to the patient. #J-18808-Ljbffr
