Overview
Registered Nurse – Geriatric travel assignment in Saint Helena, California. Working in a Short Term Acute Care Hospital setting starting 09/28/2026 for 13 weeks. Weekly pay range: $2,785–$2,934 with guaranteed hours of 40. Day shift, typically 8:00 AM–4:30 PM, 5x8 schedule with no set days.

Responsibilities
- Lead coordination of patient care with the multidisciplinary team to ensure timely and appropriate services for geriatric patients.
- Develop, implement, revise, and report on the case management plan from pre-admission through post-discharge.
- Act as liaison among patient, family, nursing staff, physicians, and care partners to optimize outcomes and discharge planning.
- Monitor resource utilization with clinicians, discuss treatment plans, and estimate length of stay; coordinate discharge planning and referrals.
- Focus on complex or high-risk patients, including frequent ED utilizers, chronic pain, substance use disorders, and housing instability.
- Coordinate transfers to higher levels of care when indicated and ensure continuity of information across settings.
- Ensure data collection for regulatory and accreditation requirements; participate in quality improvement activities.

Requirements
- Active Registered Nurse license in California (state of practice)
- Current Basic Life Support (BLS) certification
- Minimum two years of recent nursing experience in the field
- Epic electronic health record experience required
- Bachelor’s Degree in Nursing (BSN) preferred
- Ability to work in a hospital setting and manage a caseload across acute care and related needs
